The staff is professional keeps me updated on my relative care it a clean environment I'm happy with his care

I'm very pleased with the service that. LAKE WORTH. REHABILITATION CENTER is providing for me. The social services are very good and she always help me and solved any concerns that I have. The place is clean and have very good quality of services.

I'm very happy here. This place safe my life. I'm happy with the care that the CNA and nurses provide for me. I make so much friends here and I will recommend lake worth rehabilitation center to any body. All the staff female are very pretty and friendly. The make my day better!

I am a patient at this facility and quite content to stay. They are extremely friendly. I feel comfortable talking with all the staff (from CNA to those in the Business Office). If you ask the chef for something special you will get it un-less they are out of it; (i.e. I've asked for cheesecake and I got it with dinner most nights for weeks). "Gotta Love Chef Daniel". I love going to Activity as they do fun things. We have played Bingo, Po•ke•no and many other games. We have tye dyed and color rocks. What else can I say! Gotta Love Marilyn!! She is so funny and so sweet. She also helps you IF you have an any issues. Thank you LWRC.

I enjoy the activities center ,playing games c and socialization helps mental health during rough times. Outside garden is beautifully landscaped ,Gazebo is lovely for visits from family.

I'm a resident here. what I love about this place is that I can bright the day to all the staff and other residents. They give me the opportunity of be part in the facility as a Resident council president. And I can interact with the staff and they love to talk to me and they looking for me to come and participate in any activity and my opinion count. Also some family members are very happy because I visits their live one and I make them laugh and have a good time. I appreciated the opportunity that the organization give me to help other residents and bring new ideas to them. I love to advocate for others.

Our family is so grateful for the wonderful care our grandmother has received at Lake Worth Rehabilitation Center. From the very beginning, the staff has shown compassion, patience, and professionalism that put us at ease. As her son and granddaughter, we (Peter and Chelsea) have seen firsthand how attentive and supportive everyone has been making sure Grandma is not only comfortable, but also encouraged and motivated during her recovery. The team always takes the time to communicate with us and treat her with the dignity and kindness she deserves. How do you describe excellence in patient care "Lake Worth Rehabilitation Center." The DON Ivone is on top of all the different facets of the patient nurse experience. Since coming to Lake Worth rehabilitation Center. I had a visit from the DON at least three times per week. The staff there is wonderful! You are greeted with a smile from Nortesha as soon as you walk in! Maddie the Social Worker is kind and caring and truly cares about her patients.

I have been coming to visit my brother for over a year at this facility and I have to extend my heartfelt thanks to Nortesha Diaz! She is the first person you see as you enter - her smile and her caring personality make you feel comfortable. She definately goes above for the families. Nortesha is 120% a total asset to this organization!!! Thank you Nortesha for always making my family comfortable!

Staff are awesome. The Administrator Carolina is very approachable and supportive. I've worked at Lake Worth Rehabilitation for a month as a unit manager. I enjoy coming to work and hearing great comments from the residents and family members about the good care the residents receive.

Six months ago my dad had a terrible fall and was in and out of 5 hospitals and 4 rehab centers. It truly is a miracle that my dad was able to come home in part largely due to the care he received at Lakeworth Rehabilitation Center/ Elevate Care. From the friendly reception staff, nurses, nurse assistants, PT/OT staff and maintenance my father received excellent care. The first day my sister and I walked in to visit the property we saw 6 PT/OT staff members working with residents. A huge thank you to Rosemarie and Marjorie the charge nurses who were on top of my dad’s care and addressed all his needs in a timely manner, making all his appointments and communicating with his doctors. Pearl, Yanique and Laudy were the nurses who went above and beyond for my dad’s recovery. Gilberte, what would we have done without you; World’s best nurse assistant. The miracle workers: Jose, Wina, Christine, Owen, Dede and Claudia the therapy team. My dad arrived unable to prop himself up in bed and after 3 months he is walking 32 steps today and doing all of his exercises. I would also like to thank Maddie for all her organization skills and follow through from day one admittance to discharge. Again, we are so grateful for all of you!

Today is my dad first day at Elevate Care, the staffs was very helpful.

Every exchange with staff from entry to exit has been pleasant. I met Maddie the SW and felt immediately my friend was among people whose heart and soul were in the right place. Impeccably clean facility.

Staff was courteous and friendly. My mom seems very happy there. However, after one week, they neglected to send her to life saving dialysis treatment twice in one week, which led to her ending up in the hospital and 2 weeks later her death. Please do not send your family member there.

Truly excellent. I recently moved my dad here and the service exceeded my expectations! This is the cleanest facility I’ve ever been in! My dad is so much happier here! The staff is phenomenal. Thank you to Kevin in admissions, Nora at the front desk, Paula the DON is the BEST, John the ADON is so friendly, and all the nurses and staff that care for my dad. Thank you Marjorie for taking care of my dad in the evening. It’s so nice to have my dad somewhere safe and where people actually are happy, actually care, friendly, and always willing to help! Huge bonus is they always answer the phone and have incredible communication! Thank you. Elevate!

A dear friend of mine has been at elevate for a few months. I have never seen her in such good shape, she’s cheerful, she is well cared for the staff is friendly and helpful. When I go there it really seems like a place that is comfortable and Organized and keeps the idea of their residence well-being in the forefront. I would definitely recommend this place if you have someone that needs that kind of care.

Since Elevate Care took over the nursing care has improved. Marjorie, an evening nursing supervisor is terrific. Nortesha Diaz one of the reception staff is also outstanding. She knows my Mom and cares about her. The Activities Director, Lisa is also fantastic. There are some areas that could use improvement, but overall, we're glad my Mom is there.

Needs more help on weekends. Otherwise they dho great job.

Clean and friendly place. The place is very welcoming, you are greeted from entrance and staff are very helpful and accessible. It is easy to get answers if you have questions and whatever you need the staff gets it done right away. The residents were on the dining room and the set up looks very pretty and the staff are supervising and very attentive to the needs of residents. Staff looks like they are happy to work here.

Did a great job with my grandmother’s 100th birthday party.

Staff is so friendly to us nursing students and always appreciate the help we try to give when we are there. Not every care facility has staff like Elevate. That alone makes it better than others

The receiving personnel and entire staff were welcoming. I must commend a staff/technican by the name of Kay she treated the patient I was visiting in room #116 with respect and kept the pt informed as she did the procedure, she is awsome..Nice environment and welcoming Staff.

My 92 year old Mom gets the best care. Everyone is wonderful. They really care about the patients and their families. Mom is always clean. The facility is always clean. She really likes the food & you can tell a lot of thought and care goes into preparing and serving it. Everyone is very caring and available if you have any concerns. My Mom has been there for a year. Having a loved one in long term assisted living is hard. I find comfort in knowing I picked the right one.

I feel like this facility/staff tries their best to meet the quality and standard of care for their patients. Communication is key there. Everyone so far are great, including PT they work with my mom a lot and are patient with her.

The staff takes great care of my father. I do wish they had better monitoring systems. There is know way to tell there is an issue unless they walk in and check on the patient. Other than that, I am happy with this facility.

I was hesitant to place mom into a nursing home because she wanted to stay at home for as long as she could. It got to the point where we could no longer keep her at home; therefore, we went searching. We found Oasis. Oasis, it’s like the name states, I mean that they are amazing. It’s all about the people working there; from the front desk and the care givers, to maintenance. Everyone is very friendly, very pleasant and helpful. My mom, had around the clock aides before going to oasis. We thought she was near death and we put her in hospice care because she was no longer able to do anything. One week after being at Oasis and in their care my mom is alert, she’s smiling, she’s talking and having pleasant conversations. After not wanting to get out of bet for several months, she’s asking for physical therapy to get stronger so she can walk. I could not ask for a better place to place mom during this time of her life. I like to see the happiness in my mom’s face when I go to see her and I didn’t have that when she was at home, so thank you Oasis!!!

Wonderful facility. The staff are all very caring and work hard to ensure all the residents are comfortable, well cared for and happy. The therapy team is wonderful. I would highly recommend Oasis.

My father fell and fractured his hip. He has Alzheimer's and can be a handful sometimes. The staff at Oasis is very caring and knows how to care for people with difficult behaviors. His therapy was excellent and he recovered to come home with family. Thank you Oasis . I highly recommend you to everyone!

I'm so happy and appreciative with the care my mom has received at Oasis. She has been here for almost two years now. Everyone that works here is so friendly, helpful, and knowledgeable. They always make me feel so welcomed and at home. The facility is always very clean and well maintained. The food is really good and tasty. They have a wide selection of menu options to choose from each day. Oasis has many fun daily activities for the residents to do. There is a beautiful courtyard that has a really nice gazebo, and a section with tables and chairs. If I ever needed a short or long term facility, I would choose Oasis for myself. In my opinion, they deserve 5 stars! ★ ★ ★ ★ ★

My father was recently discharged after a 3 wk stay at Oasis Lakeworth. He was pleased/satisfied with his stay/rehab experience here. He says the staff was great and PT provided him good therapy each day, making him feel confident and comfortable by the end of his stay.

Hi, Overall I have been very satisfied with Oasis. The staff seems to be very attentive to my mother. They have been extremely cautious regarding her safety not only due to Covid but not allowing her to even try to walk on her own due to the fear of her falling. Mary from the beginning with being very attentive through multiple phone calls won our family over by making us feel the Oasis should be the nursing home of choice. She always returned our phone calls listened to us answered our questions and always seem to be there for us and told us we were no bother at all and we could call her anytime. However overall the communication needs to be addressed with the staff. Nurses station wasnt covered to transfer phone calls to my mother in laws room and that became very frustrating. Also after 8 pm to not be able to call is inconvenient and maybe later would be a better option. Marti Kleinman

Our mother arrived here from rehab and I have seen a marked improvement in the 3 weeks she has been at Oasis. They are very thorough in their ability to keep the residents risk free from Covid. Mary in admissions made the transition effortless from beginning to end. The entire staff has gone out of their way to welcome our mother with cheer and support. She has gained weight, gotten stronger with the physical therapy she has been receiving and is very happy with the care she has received. The communication has been adequate and I feel my mother is in good hands! Thank you to Oasis!

Everyone is so friendly, helpful, and knowledgeable at Oasis. The facility is very clean with spacious rooms that are also kept very clean as well. The food is very good and they have a large variety of meal options. They also have special holiday events for the residents and their families. They have a beautiful outside sitting area with walkways, benches, and a large gazebo. It's the perfect area to take your loved ones to get some fresh air and spend quality time with them outside. We are so grateful and appreciative to have found Oasis. We highly recommend Oasis for your loved ones!!!

Oasis provides the individualized care that most places do not. The facility is spotless every day. Therapy staff motivates and encourages patients to get them home quicker. They also help with applying for Medicaid if a long term stay is needed. Staff is also trained in care of people with dementia. Everyone is friendly and helpful. Please consider Oasis if you need these services for a loved one. They deserve their long time 5 Star rating from Medicare.

We were impressed with Oasis from the beginning. They were there to help and answer any questions we had. Made me feel like they were really there to make sure my father had the best care possible and was happy in his new environment.

When you are in need of a rehab or permanent care facility, you should check out Oasis. This is a great place with very a fantastic staff. My mom loves it here and is very satisfied with the service and treatment she receives. I highly recommend Oasis.

Oasis has all the qualities I was looking for my grandmother. Everyone is so professional and has a smile on their face. The entire building is absolutely spotless and has a very clean smell to it. My grandmother’s room is huge and looks more like a hotel and not a nursing home room. Every concern I had was answered and when i left i knew i left my grandmother in great hands. They made her feel at home and she is happy...just as i remembered her to be.

I wanted to take a moment to thank the wonderful staff at Oasis Health and Rehab center. Our family recently moved my Mother-in-law Anne in after a very tough year at another facility. She had been sick on off for almost a year, several hospital visits and hadn't gotten out of her wheel chair in over 8 months! She is now in daily rehab and walking again! She is winning Bingo so watch out. My family and I cannot thank the Oasis Staff enough.

I was so happy with the rehab therapy my father received from Oasis. Long story short, after an unfortunet fall my dad needed some rehab physical therapy to walk and get back to his life. Oasis not only provides the therapy as professionally as could be possible they also never missed brightening his mood while is was dealing with the process. The “homework” that was given to him was always understood and they encouraged him to do them for a speedy recovery. I can’t thank Oasis enough for a GREAT physical thereapy experience.
