Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

Specialty Programs

[review] => My sister in law became a client in Dec 2024 and they have done a great job. Right from our first tour, the staff was very nice and accommodating. Getting medications correct is a big deal to ensuring my sister in law is functioning and they got that right very quickly. The staff is always very friendly and respectful to me and my wife when we visit or call. My sister in law can be a little difficult because she wants what she wants, when she wants it, the way she wants it but they do a good job trying to appease her. The place is clean and the staff is responsive. My only suggestion would be perhaps to work on faster response time to patients ringing their buzzer. I know it is a fine balance between immediate need and just being a pain in the butt but I know it would improve life for their residents.

If you ask the chef for something special you will get it un-less they are out of it; (i.e. I've asked for cheesecake and I got it with dinner most nights for weeks). "Gotta Love Chef Daniel". I love going to Activity as they do fun things. We have played Bingo, Po•ke•no and many other games. We have tye dyed and color rocks. What else can I say! Gotta Love Marilyn!! As her son and granddaughter, we (Peter and Chelsea) have seen firsthand how attentive and supportive everyone has been making sure Grandma is not only comfortable, but also encouraged and motivated during her recovery. [review] => Six months ago my dad had a terrible fall and was in and out of 5 hospitals and 4 rehab centers. It truly is a miracle that my dad was able to come home in part largely due to the care he received at Lakeworth Rehabilitation Center/ Elevate Care. From the friendly reception staff, nurses, nurse assistants, PT/OT staff and maintenance my father received excellent care. The first day my sister and I walked in to visit the property we saw 6 PT/OT staff members working with residents. A huge thank you to Rosemarie and Marjorie the charge nurses who were on top of my dad’s care and addressed all his needs in a timely manner, making all his appointments and communicating with his doctors. Pearl, Yanique and Laudy were the nurses who went above and beyond for my dad’s recovery. Gilberte, what would we have done without you; World’s best nurse assistant. The miracle workers: Jose, Wina, Christine, Owen, Dede and Claudia the therapy team. My dad arrived unable to prop himself up in bed and after 3 months he is walking 32 steps today and doing all of his exercises. I would also like to thank Maddie for all her organization skills and follow through from day one admittance to discharge. Again, we are so grateful for all of you!

My sister and I spoke to several staff members for supervisor to administrative. They have little to no compassion, Jemely, who is DON, and the nurse supervisor Rosemarie are the worst. My mother was slowly declining during her time there. She was rushed to the emergency room because her respiratory was decreasing and found out she had pneumonia. From there, it was a perfect opportunity to bring my mom elsewhere for therapy. If she was to go back to that place, they would have surely and slowly killed her. I wish there was a zero star as an option.

My mom cried on the phone bc it took a cna almost 4 hrs to help her to the restroom. Her bed had a huge dent in it therfore she had to stuff pillows under the mattress. And I'm sorry but those chicken heads yall call cna's are a joke. I would be on the phone with my mom n all you can hear in the background is a zoo. Just obnoxiously loud. So the good reviews must be from family and friends of this ratchet place that looks like a drive to a cemetery upon arrival! Sad.

She knows my Mom and cares about her. The Activities Director, Lisa is also fantastic. There are some areas that could use improvement, but overall, we're glad my Mom is there.

[review] => My father received the worst care we've ever experienced and this facility almost killed him. He was taking care of himself until coming down with a stomach bug and moved to Elevate Care where he was receiving 30 minutes of PT and the remaining hours laying in his bed in a diaper (which he never wore before). One day he asked an orderly for something and was told to get it himself. We requested he be taken to the dining room for meals per a suggestion from dining hall staff and this never happened. If you have a family member or loved one you care about, please get them out of this facility before they kill them.

[review] => My father (who is a Marine veteran) went to Elevate Care the evening of January 12, 2023. We went to visit him the following day with a bag full of his clothing. When we arrived he was in a reclined chair which we were told would keep him from falling out of bed when he was alone.
It was our understanding from the hospital (Bethesda) they provided orders for him to have an IV to avoid dehydration as his intake of fluids was very low. I inquired why he was not on an IV and I was told they had to do lab work for the doctor to evaluate but not to worry as it was already done and they were just awaiting the results.
We also provided a large tote bag full of clothing for him to the nurse/aide who advised they needed to label his items (although we had previously labeled his belongings).
During our visit with him he was placed into a regular bed, however the nurse requested we alert her when we were leaving so someone could assure he would not fall out of bed (which we did).
The following day we found my father in his regular bed but he did not have an IV. When we inquired as to why there was still no IV, we were told they only did bloodwork but had not sent a urine sample to the lab (inapposite to what was related the prior day). But again they said not to worry as all samples had now been provided to the lab.
We stayed for several hours to visit with my father and prior to leaving we advised the nurse/aide that we were leaving and she gave me elevator code and I thought that an aide/nurse would go check on him or he would be transferred to the recline chair so he would not fall out of bed.
Later that evening (several hours after we left) we received a telephone call from Elevate Care to inform us my father is being taken to the hospital because he was found on the floor by his bed. The facility was well aware my father had a habit of falling out of bed and there was a special chair for him to be in when no one could watch him. It was negligent for them to not take the appropriate precautions to prevent my father (who by this point was very frail weighing approximately 110 pounds)from falling out of bed.
When he arrived at the hospital we were told although he did not break any bones he was severely dehydrated.
During the six days my father was in the hospital (and thereafter) we contacted Elevate Care to retrieve his belongings. We were repeatedly told someone would have to check on them and call us back. Of course no one ever called.
In the two days my father was in Elevate Care they allowed him to become severely dehydrated, fall out of bed (the two things we specifically spoke to them about) and they lost his clothing. Can you imagine what other damage they could have done if he stayed there longer.

It is easy to get answers if you have questions and whatever you need the staff gets it done right away. The residents were on the dining room and the set up looks very pretty and the staff are supervising and very attentive to the needs of residents.

[review] => My 95 year old mother was placed at Oasis ( now called ElevTe?) for rehab following a series of strokes. After it was determined that she would need nursing home care, she was placed in ‘Hospice care’ and moved to another room. Their visitation policy is very strict and would allow 1 hour/day scheduled by the ‘Social Worker’. I live in another state and when I came down to visit my mother and mover her things out of her ALF, I was able to have this poor, near death woman brought to a room for me to see as I was not permitted to visit in room. I was then notified that the visit could only be 45 minutes since she was not going outside for the visit (ridiculous) I was never told of this stipulation by the S.W. Next day, I was very upset as I wanted to be by my mother’s bedside as it was apparent to me that she was in her final hours. I then find out that there is another level of care beyond HOSPICE, called CRISIS. I was never informed of that by the S.W. and needed a Dr’s order. Luckily he arrived at the same time that I did, but she had to moved to yet another room, private, in a reclining stretcher of which she was very weak, I shudder thinking of them trying to get her in and out of bed. It took hours. By the time I got to see her she was barely conscious and passed away that evening. I spent only 45’ with her at her bedside due to the lack of attention to my mother’s deteriorating condition by staff, and with little to no help from the S.W. whom I never met, as she maintained her lack of empathy, sensitivity or helpfulness in this very difficult time. I feel as though my mother suffered and it haunts me. Then her diamond ring, which had been stuck on her finger, was placed at her bedside after her death and ‘lost’. Then to top it off, the administrator, on her way out, told me how nuch everybody ‘Loved my mother and had lots of friends , ‘ which really upset me because she obviously never laid eyes on her. The only shining star, was the precious, receptionist, who keeps Oasis under guard while she is there. So sweet! The activity people and rehab people are great too.

not put your love ones in that place. My mother was sent there for rehabilitation after series of strokes and the experience was not very pleasant. I used to spend hours on the phone with her because none of the nurses would respond the CALL-LIGHTS. I am talking about 4-6 hours at night. They would yell at her not knowing that I am on the phone when they finally decide to show up. She was also not getting her daily physical therapy because the therapist would walk into the room and the leave. questioned them they told me that my mother was extremely confused after they had confirmed EVERYTHING that mother told me about the occupational therapist since she was always nice to her. I was like… you just confirmed everything else? Guess what? The director got so angry and started being disrespectful. These people need to be audited …. The level of care is nonexistent and the office staff always pushing for long-term care . DO NOT SEND YOUR FAIMLY THERE!

[review] => If you are looking for a nursing home for a loved one stay away from Oasis Health & Rehab shown below. We had to resort to putting a loved one in there and had to pay 1 month in advance before she could be admitted. We were told that if the patient is not mobile we could visit her in the room otherwise they would bring them to the lobby for the visit. I went for my visit, they were all confused and after approximately 20 minutes they brought my friend out to the lobby in a bed hair a mess and barely dressed. She lived in there for 8 days and passed away.
We have been waiting for over a month and a half for the refund and get treated like we’re bothering them every time we call to see why it’s taking so long……….this is not a good place for your loved ones.

[review] => I was hesitant to place mom into a nursing home because she wanted to stay at home for as long as she could. It got to the point where we could no longer keep her at home; therefore, we went searching.
We found Oasis. Oasis, it’s like the name states, I mean that they are amazing. It’s all about the people working there; from the front desk and the care givers, to maintenance. Everyone is very friendly, very pleasant and helpful.
My mom, had around the clock aides before going to oasis. We thought she was near death and we put her in hospice care because she was no longer able to do anything. One week after being at Oasis and in their care my mom is alert, she’s smiling, she’s talking and having pleasant conversations. After not wanting to get out of bet for several months, she’s asking for physical therapy to get stronger so she can walk. I could not ask for a better place to place mom during this time of her life. I like to see the happiness in my mom’s face when I go to see her and I didn’t have that when she was at home, so thank you Oasis!!!

She was refused showers. She needed to constantly ask for water. A small cup would be given. After a week or so she needed to be put on an IV (at our insistence) because she was so severely dehydrated. On numerous occasions she wasn’t given proper meals. Not to mention that the food was inedible. The worst thing was that they didn’t take out her staples from her surgery until well after the two weeks that was recommended by the surgeon. (Again it was only addressed after our insistence). The only positive note was the occupational therapist, Linda, was amazing. She was the only person to treat my mother like a human being. I’m very disappointed in the care at this facility. It’s disgusting how they treat people.

Overall I have been very satisfied with Oasis. The staff seems to be very attentive to my mother. They have been extremely cautious regarding her safety not only due to Covid but not allowing her to even try to walk on her own due to the fear of her falling.
Mary from the beginning with being very attentive through multiple phone calls won our family over by making us feel the Oasis should be the nursing home of choice. She always returned our phone calls listened to us answered our questions and always seem to be there for us and told us we were no bother at all and we could call her anytime.
However overall the communication needs to be addressed with the staff. Nurses station wasnt covered to transfer phone calls to my mother in laws room and that became very frustrating. Also after 8 pm to not be able to call is inconvenient and maybe later would be a better option.

The food is very good and they have a large variety of meal options. They also have special holiday events for the residents and their families.
They have a beautiful outside sitting area with walkways, benches, and a large gazebo. It's the perfect area to take your loved ones to get some fresh air and spend quality time with them outside.

He does anything to make you uncomfortable and is an egotistical horrible man. I have been here close to a week nothing has been done properly or professionally. Most of the nurses are rude and don’t care about your needs and the nicest are the CNAs but they are under staffed and over worked. If you need care and assistance and a place you can comfortably live don’t come here. This place is a horror- 65 pages of rules and regulations that would be more. Suited for a prison. My handbag was searched several times to see what I was hiding which is ridiculous and invading my privacy. Don’t comeput anyone you care about near this place. I have had to be in a nursing facility for16 years and never have I come across such a disgusting place. Obviously I will leave here as soon as I can find a decent place with a long term bed preferable out of West Palm Beach. It is amazingly expensive and living on social Security and in nursing’ homes which most take close to 90 percent of your compensation so life is not easy and to be in such a miserable facility as Oasis who deserve nothing watch out.
